Although it is usually best to have a lawyer talk to you about the details of your case, there are free forms you can find to help you. Beware of the websites that try to sell you forms. Some sites will sell you forms that you can get for free from the court clerk, and you might find that you have paid a lot of money for forms that you don't know how to fill out.

LawNY created some online programs that ask you questions to help you fill out forms. You can find those forms on LawHelp/NY.

The New York courts have created online programs that can help you fill out legal forms, too. Some of these "DIY" programs were made with LawNY's help. See a full list here, on NY CourtHelp. (link) The programs you can find there include these:

The New York State Unified Court System partnered with Legal Assistance of Western New York (LawNY), Legal Services NYC, LawHelp/NY, and Pro Bono Net in a project which was funded by the Legal Services Corporation (LSC) and the State Justice Institute (SJI). The Chicago-Kent College of Law provides the A2J Author® software used to create these programs, and the National Public Automated Documents Online (NPADO) Project of Pro Bono Net provides servers and Internet support.
